Completed attendance forms have been submitted to Beth Israel Medical Center for review. Please be sure to check your email for a message from Beth Israel, containing a link to the online evaluation. CME certificates will automatically be generated and ready for you to print upon completion of the evaluation.
In order to receive your certificate, you must complete and return the following before leaving the event:

ACCREDITATION STATEMENT
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of Beth Israel Medical Center and Continuum Center for Health and Healing. Beth Israel Medical Center and St. Luke’s and Roosevelt Hospitals are accredited by the ACCME to provide continuing medical education for physicians.
CREDIT DESIGNATION STATEMENT
